29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: The employer failed to inspect the mobile scaffold (scissor lift) for defects and violations of safety and health requirements prior to allowing employees to use it while they were erecting steel on a commercial construction remodel. The most recent example of this was found on the job site located at 3301 S. Jeffers St. North Platte, NE 69103 where the employer had employees using a SLH model # 2584 scissor lift (mobile scaffold) with inadequate guard railing to connect steel roof trusses to horizontal I beams on a commercial construction site without the use of alternate forms of fall protection.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. The employer failed to protect employees from fall hazards associated with working off a scissor lift/mobile scaffold while they were erecting steel on a commercial construction remodel. The most recent example of this was found on the job site located at 3301 S. Jeffers St. North Platte, NE 69103 where the employer had employees using a SLH model # 2584 scissor lift (mobile scaffold) with inadequate guard railing to connect steel roof trusses to horizontal I beams on a commercial construction site without the use of alternate forms of fall protection.
