29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about January 27, 2015; in the fabrication department, the JD Squared Inc. HP-100 (S/N: 120026) horizontal press had 3-points of contact disks which would lay on the table surface. The disks were not secured and could protrude up and make contact with the part being bent exposing employees to struck-by hazards.
