29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.:   (a)  Facility, By-Products Area:  On or about February 9, 2015, the sump pump pit (decanter drain) was not guarded by a cover nor a guardrail.  The existing steel grating that was over the pit had been moved off of the pit.  This grate was removed from the pit to allow a steam siphon to be placed into the pit to drain the water.  An employee was injured when he stepped into this 18 inch deep pit which was full of hot water.  The employee received second degree burns.
