29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Worksite located at 9805 S 219th East Ave. Broken Arrow, OK 74014. On or about February 17, 2015 and at times prior thereto, adequate guarding was not in place for the Amada press brake(s) being used in the metal fabrication shop. The employees operating the presses were exposed to the hazard of being caught in the point of operation.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:   Worksite located at 9805 S 219th East Ave. Broken Arrow, OK 74014. On or about February 17, 2015 and at times prior thereto, electrical panel boxes were found that had open knockouts that had not been effectively closed. The employees were exposed to electrical shock.

On or about February 17, 2015 and at times prior thereto, electrical panel boxes were not labeled to identify each branch circuit's purpose. The employees were exposed to the hazard of electrical shock.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:   Worksite located at 9805 S 219th East Ave. Broken Arrow, OK 74014. On or about February 17, 2015 and at times prior thereto, the employer did not develop, implement or maintain a written hazard communication program.  Employees at the facility use chemicals such as, but not limited to, anti-splatter welding spray exposing then to skin and eye irritant hazards.
