29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.) Shop area - On or about February 13, 2015, employee(s) were required to use the Queen City bench grinder and the 10 inch diameter bench grinder to grind steel and aluminum. Safety guards (tongue guards) for the abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel. Employees were thereby exposed to the hazards of flying particles in the event the grinding wheel shattered.

29 CFR 1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report. Throughout facility - On or about February 13, 2015, a log for 2013, 2014, and up-to-date 2015 of all Work-Related Injuries and Illnesses (OSHA Form 300) and/or Summary of all Work-Related Injuries and Illness (OSHA Form 300-A), and/or Injury and Illness Incident Report (OSHA Form 301) or equivalent forms were not kept by the establishment.
