29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports:  On the second tier of a tubular scaffold, on or about February 18, 2015, an employee was exposed to a fall hazard of approximately 11 feet to the paved ground below when working on a scaffold that had 2 of a possible 5 planks installed while skimming the east side wall of a commercial building.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    On a tubular scaffold, on or about February 18, 2015, employees were exposed to fall hazards of approximately 11 feet in height to the paved ground below from climbing the scaffold end frames and cross braces to access the scaffold to skim the east side wall of a commercial building.

29 CFR 1926.451(h)(2)(ii): A toe board was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:  On the first tier of a tubular scaffold, on or about February 18, 2015, an employee was exposed to a struck by hazard from items such as but not limited to a masonry flat top trowel when working below the second tier of an approximately 11 feet high scaffold that did not have toe boards installed while skimming the east side wall of a commercial building.
