29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  (a) East side of sewing area, 220 Volt/ 100 Amp panel: On or about February 18, 2015, and at times prior thereto, employees were exposed to electrical shock hazards from an open electrical knockout measuring approximately 2 inches on an electrical panel, while resetting breakers in the panel.  (b) West side of sewing area, 110 Volt/ 20 Amp 2 inch by 4 inch receptacle: On or about February 18, 2015, and at times prior thereto, employees were exposed to electrical shock hazards from an open electrical knockout measuring approximately 1 inch on an receptacle, while plugging in fans and/or tools.
