Basement, Morgue Room - The employer did not provide appropriate eye wash facilities within the immediate work area for emergency use.  Employees regularly worked with Kelco Kelfirm 30 Arterial Embalming Fluid.  The safety data sheet for this chemical stated that it contained 29 percent formaldehyde.  The water tank on the Bradley On-Site Gravity-Fed self-contained eyewash unit was almost empty.  Observed On or About February 13, 2015
