29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  Employees were exposed to smoke inhalation and burn hazards from:  a) the exit route between the living room and bedrooms 4, 5, 6, and 7 in the Assisted Living Building (building 2) was not marked to indicate the nearest exit discharge by the patio.  b)  the exit route between the living room and bedrooms 11, 12, 13, and 14 in the Assisted Living Building (building 2) was not marked to indicate the nearest exit discharge by the patio.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  Employees in the Memory Building, building 1, were exposed to electrical shock and burn hazards from:  a)  an orange flexible cord that was plugged into an outlet in the food pantry and ran along the floor until terminating at the pantry freezer's plug end.  b)  a white flexible cord that was plugged into an electrical outlet in the kitchen and terminated at the plug end of the proximity door alarm system's annunciator.  c)  a yellow flexible cord in series with a three way outlet extender and a black flexible cord in the Garage and terminated at the plug end of a refrigerator.
