29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: a) Metalcraft Enterprises, Inc. @ 202 Industrial Drive, New Haven, MO 63068: On and before February 18, 2015, the employer did not ensure that point of operation hazards were guarded on the dual head robotic welding cell. The pneumatic clamps which secure the cb antenna mounting bracket were not guarded and thereby created amputation hazards to the operator. b) Metalcraft Enterprises, Inc. @ 202 Industrial Drive, New Haven, MO 63068: On and before February 18, 2015, the employer did not ensure that point of operation hazards were guarded on the dual head robotic welding cell. The hydraulic clamps which secure the upper and lower mirror mounting bracket were not guarded and thereby created amputation hazards to the operator. c) Metalcraft Enterprises, Inc. @ 202 Industrial Drive, New Haven, MO 63068: On and before February 18, 2015, the employer did not ensure that point of operation hazards were guarded on the dual head robotic welding cell. The hydraulic clamps which secure the bent tube frames were not guarded. On February 16, 2015 an operator loaded parts for the 8961 and 8962 mirror frames when a hydraulic clamp, estimated at 600 pounds per square inch, was actuated and caught the tip of the operators right thumb. The operator suffered a bone fracture and partial amputation of his right thumb.
