29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: a) The employer's evaluation of the wood pellet storage bin did not include the potential wood pellet engulfment hazard, wood pellet carbon monoxide generation or creating a combustible dust cloud during cleaning operations. Employer obligations with identified permit-required confined spaces include, but are not limited to: notification to employees by posting signs or equally effective means, determination if employees will enter permit-required confined spaces and, if employees will enter the permit-required confined space, development of an entry program meeting the requirements of 29 CFR 1910.120(c)(5), (c)(7) or (d).
