The step-across distance from the nearest edge of ladder to the nearest edge of equipment or structure shall be not more than 12 inches, or less than 2 1/2 inches. a. At the workplace located at 150 East Gay Street, Columbus, Ohio, for access to the freight elevator pit, the step-across distance from the hoistway sill to the fixed ladder was approximately 16.5 inches, thereby exposing employees to an approximately 12 feet fall hazard. On February 5, 2015, an employee fell approximately 12 feet while exiting the freight elevator pit, resulting in serious injury.
