29 CFR 1910.215(a)(1): Abrasive wheel(s) used on grinding machinery were not used with safety guard(s): (a) WELD SHOP - Employee(s) used a DeWalt D28402 portable grinder (SN: 073035), and the guard for the grinder had been removed, thereby exposing the employee(s) to serious injury, on or about February 13, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) WELD SHOP - Employees used a TCM FVG25N1 forklift to support material while it was cut and/or welded. The employer had welded clevis style hooks to the forklift cab rollover support structure and also to the fork carriage for use as restraints to keep the forks from falling while supporting the loads. The employer had not obtained the manufacture's approval prior to performing the modifications to the equipment, thereby exposing employees to possible injury, on or about February 13, 2015. (b) WELD SHOP - Employees used a Hyster H110XL forklift to support material while it was cut and/or welded. The employer had welded clevis style hooks to the forklift cab rollover support structure and also to the fork carriage for use as restraints to keep the forks from falling while supporting the loads. The employer had not obtained the manufacture's approval prior to performing the modifications to the equipment, thereby exposing employees to possible injury, on or about February 13, 2015. (c) WELD SHOP - Employees used a Yale GP-080 forklift to support material while it was cut and/or welded. The employer had welded clevis style hooks to the forklift cab rollover support structure and also to the fork carriage for use as restraints to keep the forks from falling while supporting the loads. The employer had not obtained the manufacture's approval prior to performing the modifications to the equipment, thereby exposing employees to possible injury, on or about February 13, 2015.
