The employer had welded clevis style hooks to the forklift cab rollover support structure and also to the fork carriage for use as restraints to keep the forks from falling while supporting the loads.  The employer had not obtained the manufacture's approval prior to performing the modifications to the equipment, thereby exposing employees to possible injury, on or about February 13, 2015.  (b)  WELD SHOP - Employees used a Hyster H110XL forklift to support material while it was cut and/or welded.  The employer had welded clevis style hooks to the forklift cab rollover support structure and also to the fork carriage for use as restraints to keep the forks from falling while supporting the loads.  The employer had not obtained the manufacture's approval prior to performing the modifications to the equipment, thereby exposing employees to possible injury, on or about February 13, 2015.  (c)  WELD SHOP - Employees used a Yale GP-080 forklift to support material while it was cut and/or welded.  The employer had welded clevis style hooks to the forklift cab rollover support structure and also to the fork carriage for use as restraints to keep the forks from falling while supporting the loads.  The employer had not obtained the manufacture's approval prior to performing the modifications to the equipment, thereby exposing employees to possible injury, on or about February 13, 2015.
