On or about February 10, 2015, employees were using a JLG 6042 that was not specifically designed by the manufacturer for such use to lift personnel baskets.  Employees were being exposed to crushing hazards.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  a. On or about February 10, 2015, the employees were using a job-made basket that was not attached to the forks and the fork-lift was moved horizontally while the basket was occupied.  This exposed the employees to fall and crushing hazards up to 14 feet.
