On or about February 10, 2015, the supervisor for Noel Hughes & Associates, Inc. was working from the JLG lull with a job made basket. The employee did not have on a body belt and lanyard attached to the basket. The employee exposed himself to a 12 feet fall hazard.

On or about February 10, 2015, employees of Noel Hughes & Associates, Inc. were installing a metal roof system. The employees were not protected from falling 14 feet by the use of a guardrail system, safety net system, personal fall arrest system or any other type of fall protection method.
