29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2). F/V Bountiful, OSI dock, Ammonia storage tanks: On or about 6 February 2015 and times prior thereto, the Ammonia tank deck catwalk had an unguarded opening measuring 61" long x 12" wide. This condition exposed employees to a 100 " fall hazard.

29 CFR 1915.89(n)(2): The employer did not ensure that each lock and tag was uniquely identified for the purpose of controlling hazardous energy and was not used for any other purpose. F/V Bountiful, OSI Dock, Engine Room: On or about 6 February 2014 and times prior thereto, a danger tag was affixed to an operating header blade sharping machine. This was not following the administrative procedures of Lock-out/Tag-plus system.
