F/V Bountiful, OSI dock, Ammonia storage tanks:  On or about 6 February 2015 and times prior thereto, the Ammonia tank deck catwalk had an unguarded opening  measuring 61" long x 12" wide.  This condition exposed employees to a 100 " fall hazard.

This was not following the administrative procedures of Lock-out/Tag-plus system.
