On or about February 18, 2015: Employees working in or near the  paint/pre-wash area of the facility are subjected to entrapment hazards.  The emergency exit door had become stuck and could not be opened due to rust build up on the outside of the door.

29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet):   On or about February 18, 2015: Employees working near the grinding area in the facility are subjected to entrapment hazards.  The door in this area did not have a "Not an Exit" sign posted on it.  This door is to a hallway that leads to an area that is no longer being used by the employer and is not an exit.

29 CFR 1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color:   On or about February 18, 2015: Employees working in or near the  paint/pre-wash area of the facility are subjected to fire and entrapment hazards.  The employer did not ensure that the emergency exit sign over the exit door near the pre-wash booth was illuminated.

On or about February 18, 2015: Incident involving forklift operator where the 55 gallon drum of pre-wash solution was ruptured by the passing vehicle.  The operator was not re-trained until the OSHA inspection was performed.

The tongue guards were missing or the distance between the abrasive wheel and the tongue guards exceeded one fourth inch.
