29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable gas are being used on the jobsite: a) Alumni Hall on WPI Campus, Worcester, MA: On or about February 18, 2015, there was more than 5 gallons of gasoline (in 3 metal gas cans at the roof top work location) present without having a fire extinguisher available.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Alumni Hall on WPI Campus, Worcester, MA: On or about February 18, 2015, employees were removing snow, with a snow blower, from the roof of this building, to facilitate construction activity, more than 18 feet above the ground below without any fall protection. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
