On or about February 12, 2015, at the billboard, the employee replacing the billboard sign was not protected from falling 13 feet by the use of a guardrail system, safety net system, personal fall arrest system or any other type of fall protection method.

29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a. On or about February 12, 2015, at the billboard, the employee using a JLG aerial lift to go up and down from the cat walk was not wearing a body belt and lanyard attached to the basket. He was being exposed to a 13 feet fall hazard.
