The equipment was listed for household use and instructions directed it be plugged directly into a wall outlet.      Notice 01, Item 001, 1910.305(g)(1)(iv)(A) as required by 29 CFR 1960.8(b):  The employer failed to ensure appliances were plugged directly into a wall outlet.  The employer was cited for a violation of the same standard on 2/14/2014, Inspection # 957822, 03/18/2014 of final order, and was abated by plugging appliances directly into wall outlets.
