29 CFR 1926.1126(f)(2): The employer required the use of respirators and did not institute a respiratory protection program in accordance with the Respiratory Protection standard, 29 CFR 1910.134, which covers each employee required to use a respirator:  a) A written respirator program with worksite-specific procedures was not developed at the Woodbridge Energy Center construction project at 1070 Riverside Dr. in Woodbridge NJ, where tight-fitting air-purifying respirators and/or powered air-purifying respirators were provided & required by the employer.  Flux core welding inside of welding enclosures at HRSG Units A+B, involving stainless steel of 9% Chrome, had some measured exposures above the Permissible Exposure Limit (PEL) of 5 micrograms per cubic meter averaged over 8 hours (ranging from 5.4 - 15.2).  Site-specific procedures on medical evaluations & fit-tests, respirator maintenance and regular evaluation of program effectiveness were not documented in a written respirator program.    Condition(s) noted on or about 2/18/2015.
