29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:    On or about February 19, 2015, employer did not ensure that employees operating the boom lift were secured to the basket. Employee was replacing a parking lot light ballast but, he was not wearing fall protection equipment. This condition exposed employee to fall hazards exceeding 4 feet.
