29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. On or about February 18, 2015, employer did not ensure to provide training to employees. Employer did not ensure that employee operating a scissor lift had sufficient knowledge of the mechanical operation of the machine to recognize a malfunction or potential malfunction. This condition exposed employee to falls exceeding 10 feet.
