F/V Clipper Surprise, OIS Dock, Baiter are of vessel: On or about 8 February 2015 and times prior thereto, the emergency exit lights in the baiter area was inoperative. This condition exposed employees to delays in existing the vessel resulting in smoke inhalation and fire hazards.
