29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification. a) 1930 Terrier Ave, Dam Neck Annex, Virginia Beach, VA 23461: On or about February 9, 2015, the employer did not ensure extension cords were used in accordance with the listing and labeling in that three extension cords were daisy chained together to supply power tools used by workers on the roof, and two extension cords were daisy chained together to supply a heater on the ground level. No abatement certification or documentation required.
