29 CFR 1926.760(b)(3): 29 CFR 1926.760(b)(3): Each connector working at heights over 15 and up to 30 feet above a lower level was not provided with a personal fall arrest system, positioning device system , or fall restraint system, and/or was not wearing the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section: Worksite located at 4000 Carriage Way, Weatherford, OK 73096. On or about February 20, 2015, an employee was observed working at a height over 15 feet and was not provided with a fall protection harness. The employee was exposed to a fall hazard of over 15 feet.
