29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: a. On or about February 19, 2015, at N. Nebraska Avenue and e. Lake Avenue, Tampa, FL, employees were utilizing a bucket truck with an expired leveling cable, which keeps the bucket from falling. The manufacturer's stamp on the leveling cable was dated November 2003, and, according to the manufacturer, the leveling cable is good for 8 years or 8.000 hours, whichever comes first. Employees had not been provided effective information and training on recognizing the manufacturer's expiration date on the leveling cables and avoiding an unsafe condition by replacing the leveling cables prior to the expiration date or removing the bucket trucks from service prior to expiration of the leveling cables.
