29 CFR 1926.416(a)(1): Employee(s) were permitted to work in proximity to electric power circuits that were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a) North Vault, NV-4, Switch 11A - On or about 02/10/2015, two workers were exposed to a 13Kv arc flash and were seriously injured as a result of entering the 11A switch and contacting live parts while the switch was known to be energized.
