29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) YZ Enterprises on or about February 13, 2015, at the address of 1930 Indian Wood Circle, Maumee, OH: The employer failed to ensure the rotating sealing and cutting tool contained on the Fuji Wrapper Machine (model FW 3200) was adequately guarded. An operating employee was exposed to amputations hazards after reaching into the output end of the machines enclosure to unjam plastic wrapping material near the point of operation. When performing this task, the operators hand was within the proximity of the rotating sealing and cutting tool.
