29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a) Production Workshop Area - On or about February 20, 2015, an employee used a Dayton bench grinder which had a work rest out of alignment, exposing the employee to injury in the event of the material under work became jammed in the grinder and forcibly dislodged.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a) Production Workshop Area - On or about February 20, 2015, an employee used a Dayton bench grinder that had a missing tongue guard, exposing employees to injury from flying debris.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:  a) Throughout Production Work Area - On or about February 20, 2015, employees are performing production work using equipment which is serviced by outside contractors, but the methods and procedures of contractor energy control procedures have not been provided to the individual employees to ensure they do not conduct activities which would reduce the effectiveness of the contractor's control processes and lead to employee injury from unexpected system startup.
