The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. a) Finishing Department - On or about June 2014 - February 10, 2015, a finishing/sanding employee was required by the employer to wear a 3M 8511 N95 respirator at all times while in the workplace. Prior to 2/10/15, the employee was last fit-tested in January 2013. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.178(o)(2): Load(s) were being handled which exceeded the rated capacity of the industrial truck(s) a) GENK trailer loading area - On or about 2/19/15, employees used Hyster CL 100 powered industrial trucks (equipped with long forks) to load the GENK trailers. Each load of Corian sheets weighed 6672 lbs and the Hyster's load capacity was 4700 lbs at a 73" load center. NO ABATMENT CERTIFICATION REQUIRED
