29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. (a)Avilas Hardwood Flooring @ 11286 Colony Circle, Broomfield, CO 80021: On and before February 20th, 2015, the employer failed to ensure that employees engaged in work 6 feet or more above lower levels were protected from falling by an adequate fall protective system. An employee was working on a balcony cutting wood flooring. This condition exposed employees to fall hazards of approximately 10 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards: (a)Avilas Hardwood Flooring @ 11286 Colony Circle, Broomfield, CO 80021: On and before February 20th, 2015, the employer did not provide a training program for each employee who might be exposed to fall hazards. An employee was working with power tools on an unguarded balcony elevated 10' from the ground without adequate fall protective system. This condition exposed employees to fall hazards.
