29 CFR 1910.22(b)(1): 29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. a) MetalTek International, Inc., DBA: Carondelet Foundry Corporation @ 8600 Commercial Blvd., Pevely, MO 63070: On and before March 23, 2015, the employer did not ensure that the aisle along the No Bake Floor was free from hazards. Employees poured excess molten alloy into open-top, pig molds that were located along an aisle used by employees not equipped with necessary levels of protective equipment to protect them against accidental contact with molten alloy in the event of a trip or fall.

29 CFR 1910.23(b)(1): Every wall opening from which there is a drop of more than 4 feet was not guarded: a) MetalTek International, Inc., DBA: Carondelet Foundry Corporation @ 8600 Commercial Blvd., Pevely, MO 63070: On and before March 23, 2015, the employer did not ensure that the retort pit was fully guarded. This condition exposed employees to a fall hazard of approximately 5 feet when they removed a chain guardrail to perform an alloy pour procedure.

29 CFR 1910.184(c)(6): Slings were not securely attached to their loads: a) MetalTek International, Inc. DBA Carondelet Foundry Corporation @ 8600 Commercial Blvd., Pevely, MO 63070: On and before March 23, 2015, chain slings were improperly attached to 55 gallon drums via punctured holes in the side of the drum. Employees in the induction furnace area used a pick axe to puncture drums which contained raw metals. This condition exposed employees to struck-by hazards when they used an overhead crane to lift the loaded drums to the induction furnace.
