DBA Carondelet Foundry Corporation @ 8600 Commercial Blvd., Pevely, MO 63070: On and before March 23, 2015, chain slings were improperly attached to 55 gallon drums via punctured holes in the side of the drum. Employees in the induction furnace area used a pick axe to puncture drums which contained raw metals. This condition exposed employees to struck-by hazards when they used an overhead crane to lift the loaded drums to the induction furnace.
