29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : Engine Space: On or about February 6, 2015 and times prior thereto, employees had not had a yearly audiogram. This condition exposed employee to verifying if they have any hearing loss and the necessity of wearing hearing protectors.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): (a) Galley: On or about February 6, 2015 and times prior thereto, a fan that was less than 7 feet from the floor had an area employees could contact the active fan belt. This exposed employees to laceration/amputation hazards to their hands.
