29 CFR 1926.651(k)(2): Where the competent person finds evidence of a situation that could result in a possible cave-in, exposed employees shall be removed from the hazardous area until the necessary precautions have been taken to ensure their safety: a) 6th & Lombard St, Philadelphia, PA - The employee was working in a trench approximately 15 feet long by 3 feet wide and 11 feet deep in which the competent person did not make a determination of a possible cave-in hazards and did not install a protective shield.This hazard exposed the employees to cave-in/struck by hazards, on or about February 6, 2015.
