29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      a) Workshop - Operator was exposed to amputation and crushing injuries when removing material from the point-of-operation of an Engel Industries - Fold & Shear - machine.  For The machine was operated without a program in place for the control of hazardous energy on, or about, 02/17/2015.        Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Workshop -The employer did not provide machine guarding to protect operator(s) in the machine area from hazards at the point-of-operation.       Operator is exposed to amputation and crushing injuries when removing metal from the point-of-operation. Operator is within two to six inches from the unguarded point-of-operation.      The Engel Industries, Fold & Shear 5.3.2., machine did not have a guard method in place for the shear, exposing the operator(s) to the point-of-operation; on, or about, 02/17/2015.         Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
