29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) West wall, between office and shop: On or about February 05, 2015, employees were exposed to an electrical hazard, while plugging in a 2 bulb, 4 foot, fluorescent light fixture, where the extension cord being used to power the light fixture was missing a grounding pin. b) Next the Boss 342 CNC Lathe: On or about February 05, 2015, employees were exposed to an electrical hazard, while plugging in a 2 bulb, 8 foot, fluorescent light fixture, where the extension cord being used to power the light fixture was frayed at the plug end, which exposed the extension cord's internal wiring and lacked proper strain relief.
