29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about February 16, 2015, the employer failed to ensure that effective guarding was in place to protect employees from the stamping area of the Haas Room hot stamper. The two hand controls did not function properly to prevent employees from coming into contact with the pinch point during operation.       b) On or about February 24, 2015, the employer failed to ensure that effective guarding was in place to protect employees from the moving machine parts and stamper of the Kurz hot foil stamping equipment. A trip bar was present on the input side of the equipment, however it did not prevent employees from coming into contact with other hazardous areas including pinch points and rollers.   c) On or about February 24, 2015, the employer failed to ensure that effective guarding was in place to protect employees from the pinch point of the closing relay of the screen printing equipment. The equipment is operated by foot pedal, allowing employee hands to come into contact with the closing ram area of the contact relay which initiates the printing action of the equipment.
