29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about 02/24/2015; at the jobsite on 1044 Nicholson Dr., the employer failed to ensure that adequate fall protection was being worn by employee's working on the permanent roof awning. The employees were working at heights greater than ten feet.

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level: On or about 02/24/2015; at 1044 Nicholson Dr. in front of the building, the employer failed to secure an extension ladder used by employees for access to an upper landing. The employees ascending and descending the ladder were exposed to the hazard of falling from heights greater than six feet whenever the ladder would slip from one side to the other.
