29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded:  a) In the peeler area, on or about and at times prior to February 26, 2015, the employer did not ensure an opening in the floor, that was in place to allow for a conveyor system to remove wood chips and debris, was properly covered to prevent employees from stepping or falling into it, exposing employees to caught-in hazards.
