29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.    a) On or about, February 19, 2015, while working to reattach the jog motor chain for the hydraulic three tier "G" press, located in the Roll Goods Department at 1833 East Market St. Akron, Ohio 44305, hazardous energy control procedures(lockout/ tagout) were not utilized by the maintenance employee exposing him to an amputation hazard.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees.  a) On or about, February 19, 2015, while working to reattach the jog motor chain for the hydraulic three tier "G" press, located in the Roll Goods Department at 1833 East Market St. Akron, Ohio 44305, a lock or tagout device was not affixed to the press jog motor, by the maintenance employee, exposing him to an amputation hazard.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed.    a) On or about February 24, 2015, at the facility located at 1833 East Market St., authorized employees who perform hazardous energy control were not periodically inspected for competency with regards to lockout/ tagout since March 18, 2013.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s).    a) On or about, February 24, 2015, while operating the hydraulic three tier "A" press, located in the Roll Goods Department at 1833 East Market St. Akron, Ohio 44305, the press operators are exposed amputation hazards when they reach into the press  tiers to loosen the pressed material after the cycle is complete. Additionally, operators are exposed to an amputation hazard when they are free to move around the press after the cycle is initiated and the three tiers of the press begin to close.     b) On or about, February 24, 2015, while operating the hydraulic three tier "B" press, located in the Roll Goods Department at 1833 East Market St. Akron, Ohio 44305, the press operators are exposed amputation hazards when they reach into the press  tiers to loosen the pressed material after the cycle is complete. Additionally, operators are exposed to an amputation hazard when they are free to move around the press after the cycle is initiated and the three tiers of the press begin to close.     c) On or about, February 24, 2015, while operating the hydraulic three tier "C" press, located in the Roll Goods Department at 1833 East Market St. Akron, Ohio 44305, the press operators are exposed amputation hazards when they reach into the press  tiers to loosen the pressed material after the cycle is complete. Additionally, operators are exposed to an amputation hazard when they are free to move around the press after the cycle is initiated and the three tiers of the press begin to close.    d) On or about, February 24, 2015, while operating the hydraulic three tier "D" press, located in the Roll Goods Department at 1833 East Market St. Akron, Ohio 44305, the press operators are exposed amputation hazards when they reach into the press  tiers to loosen the pressed material after the cycle is complete. Additionally, operators are exposed to an amputation hazard when they are free to move around the press after the cycle is initiated and the three tiers of the press begin to close.     e) On or about, February 24, 2015, while operating the hydraulic three tier "E" press, located in the Roll Goods Department at 1833 East Market St. Akron, Ohio 44305, the press operators are exposed amputation hazards when they reach into the press  tiers to loosen the pressed material after the cycle is complete. Additionally, operators are exposed to an amputation hazard when they are free to move around the press after the cycle is initiated and the three tiers of the press begin to close.     f) On or about, February 24, 2015, while operating the hydraulic three tier "F" press, located in the Roll Goods Department at 1833 East Market St. Akron, Ohio 44305, the press operators are exposed amputation hazards when they reach into the press  tiers to loosen the pressed material after the cycle is complete.
