29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    a. Site - On or about February 24, 2015, fall protection was not utilized by employees performing snow and ice removal from the roof area and exposed to falls of over 39 feet.     Dwight & Sons Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 909933, citation number 1, item number 2a and was affirmed as a final order on 7/15/2013, with respect to a workplace located at Mountainside at Attitash Unit #2, Route 302, Bartlett, NH 03812.
