29 CFR 1910.178(p)(1): Powered industrial truck with defect(s) or in any way unsafe had not been withdrawn from service until restored to safe operating condition(s): a) United States Postal Service, DBA USPS @ 5800 Phantom Drive, Hazelwood, MO 63042: On and before February 12, 2015, the employer did not ensure that the Toyota Tugger, numbered M071, was available for service following an employee injury. The tugger was equipped with a modified ergonomic mat that was fashioned from a conveyor belt. The conveyor belt measured approximately 17.5 inches wide while the standing platform for the tugger measured approximately 16.5 inches wide. The difference in width between the modified ergonomic mat and the standing platform caused the modified ergonomic mat to curl up in the back and thereby, created a tripping hazard.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) United States Postal Service, DBA USPS @ 5800 Phantom Drive, Hazelwood, MO 63042: On and before February 12, 2015, the employer did not notify OSHA within 24 hours when they learned an employee was admitted to a hospital as a result of a work-related injury. The employer was aware the employee was admitted to a hospital on February 16, 2015 for an incident that occurred on February 12, 2015. The employer did not notify OSHA until February 19, 2015.
