29 CFR 1926.502(a)(2): Employers failed to provide fall protection systems that comply with all other pertinent requirements of this subpart:    The employer failed to verify the existence and use of compliant fall protection systems to protect employees from fall hazards associated with working on formwork on a commercial construction site:  a)	The most recent example of this was found on the job site located at 111 W. Harvest Dr., Lincoln, NE 68510 where the employer had employees worked near a form scaffold where the employees had stepped up onto the top 16 inches of the lower wall with inadequate guard rails and in the process exposing themselves to an eleven foot fall hazard.  b)	The most recent example of this was found on the job site located at 111 W. Harvest Dr., Lincoln, NE 68510 where the employer had an employee tying in rebar on the formwork and using a form securing bracket secured only with nails as a substitute anchor point for his positioning chains; the employee climbed the form approximately 7 feet above the scaffold and attached his positioning chains to the substitute anchor, point before he could attach his fall arrest lanyard the substitute point pulled free and the employee fell striking the railing on the scaffold below before landing on the ground 19 feet below the point he was working.
