29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards.  The employer does not protect employees from falling when working on open-sided platforms four feet or more above ground level with standard railings.   a) This violation was observed on or about  February 25, 2015, on the open-sided platform next to Extruder #17 where employees were exposed to a fall hazard of four feet when working on the platform having open-sided floors.   b)This violation was observed on or about  February 25, 2015, on the open-sided platform next to Extruder #18 where employees were exposed to a fall hazard of four feet when working on the platform having open-sided floors.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that open-sided platforms four feet or more above the ground level area guarded with standard railing (or equivalent).

29 CFR 1910.184(c)(2): Makeshift device(s), knot(s) or bolt(s) were used to shorten sling(s).  The employer does not ensure that makeshift devices, knots or bolts were not used to shorten slings.  This violation was observed on or about February 25, 2015, in the printing area where employees were exposed to stuck-by hazards when using slings that were shorten by tying knots or wrapping duct tape on the sling.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that operators do not use makeshift devices, knots, or bolts to shorten slings.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   The employer does not protect the operator and other employees in the extruder area from hazards such as those created by rotating parts.  This violation was observed on or about February 25, 2015, in the extruder area where an employee was exposed to caught-by hazards when operating extruder #16 that was not guarded to protect the operator from the rotating shaft near the control panel.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that operators and other employees in the machine area are protected from hazards such as those created by rotating parts.
