29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a. In the minting area, the Kuldeep Rolling mill did not have guards in place at the point of operation to prevent employees from having any part of their body in the danger zone.

29 CFR 1904.7(b)(3): How do I record a work-related injury or illness that results in days away from work? When an injury or illness involves one or more days away from work, you must record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column. If the employee is out for an extended period of time, you must enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known.    Located at Ohio Precious Metals in Jackson, OH:    a. On the 2015 OSHA 300 log, the employer did not record the number of days away in column K for case number 15-01 on 1/30/15.

When an injury or illness involves restricted work or job transfer but does not involve death or days away from work, you must record the injury or illness on the OSHA 300 Log by placing a check mark in the space for job transfer or restriction and an entry of the number of restricted or transferred days in the restricted workdays column.  At Ohio Precious Metals in Jackson, OH:  a. On the 2014 OSHA 300 log, case number 14-08 on 10/30/14 the employer did not place a mark in column I for job transfer or restriction.  b. On the 2015 OSHA 300 log, case number 15-02 on 2/4/15 the employer did not place a mark in column I for job transfer or restriction.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  Located at Ohio Precious Metals in Jackson, Ohio:  a. On the 2015 OSHA 300 log, the employer did not include all information required in column F.     1. Case number 15-01 on 1/30/15, did not include which fingers were amputated and what caused the injury.     2. Case number 15-02 on 2/4/15, did not include what caused the laceration to the finger.  b. On the 2014 OSHA 300 log, the employer did not include all the required information in column F.      1. Case number 14-08, MD on 10/30, did not include the cause of the injury.
