29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) Employees were exposed to electrical hazards as there was no written lockout tagout program for machinery such as, but not limited to the Heidelberg CD74 printing press.  Employees were also using tagout rather than lockout on the Heidelberg CD74 printing press when it is capable of being locked out.

29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:  (a) Press Room: Employees were exposed amputation hazards as the Aqua Fog Turboxe fogger had greater than 1/2 inch openings to the fan blades.

29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    (a) Basement:  An employee was exposed to amputation hazards as the 10 inch Craftsman Table Saw did not have an automatically adjusting hood which completely enclosed the portion of the saw above the table and above the material being cut.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) Employees were exposed to chemical hazards as there was no written hazard communication program describing labeling of containers, safety data sheets will be obtained, and employee information and training.  Employees are exposed to Bottcherin 60 contains solvent, and Barton Solvents which contains Toluene and there was no written hazard communication program.
