Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:   a)  The Praxis Companies, LLC, 372 Rundle Rd., New Castle, Pa  16102:  Nesting area of the Warehouse:  On or about February 25th, 2015 and times prior thereto:  Employees were potentially exposed to fire hazards from the improper use of a Torpedo heater.  The Procom Magnum torpedo heater (Model PCK220VT) was utilized improperly in that combustible materials including but not limited to, wooden boards, cardboard materials and nested bathtubs were stored within the minimum clearances set forth by the manufacturer's recommendations for clearance to combustible materials.  In addition, a 55 gallon drum of kerosene was stored approximately 8 feet from the torpedo heater.

29 CFR 1910.106(e)(2)(iv)(d): Flammable liquids were not drawn from or transferred into vessels, containers, or portable tanks within a building only through a closed piping system, from safety cans, by means of a device drawing through the top, or from a container or portable tanks by gravity through an approved self-closing valve:    a.) The Praxis Companies, LLC, 372 Rundle Rd., New Castle, Pa  16102:  Nesting area of the Warehouse:  On or about February 25th, 2015 and times prior thereto: Employees were dispensing Marathon K-1 Kerosene from a drum into a container.  The liquid was dispensed from the drum through a quarter turn valve and small pipes.

The Praxis Companies, LLC, 372 Rundle Rd., New Castle, Pa 16102:  Desktop of a computer station located at the Patch line/1st inspection station:  On or about February 25th, 2015 and times prior thereto: A metal electrical receptacle box was powering equipment including, but not limited to, a Dell brand computer system and monitor.   A knockout blank was missing on the electrical receptacle box.

The metal electrical receptacle box was setting on a desk and was not firmly secured.
