29 CFR 1910.141(c)(1)(i): Toilet facilities were not provided in accordance with TABLE J-1 of this Section: a) South Building Locker Room- The employer failed to provide an adequate number of water closets for the female employees use. Of the nine water closets that were provided in the locker room only two were fully functional. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place and maintained in a legible condition: a) Facility Wide- The employer failed to ensure that all powered industrial trucks, such as the Toyota Tractor T11, Toyota Tractor T24, Toyota Tractor T32, Toyota Tractor T5, and the Toyota Tractor T33, were provided with nameplates with markings in a legible condition. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
