29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Rear Right, Outside Ground Level of Structure - Two workers were working on the outside window and door trim from the basket of a 40' hydraulic boom man lift, approximately 15' from the ground. One worker was wearing the required personal fall restraint/arrest system while the other worker was not, on or about 2/25/2015.
